WAIVER, RELEASE OF LIABILITY AND HOLD HARMLESS
I am an Adult (“I”) who has enrolled in a Bronzeville Fencing Academy program. In return for being accepted into a program being offered by the Bronzeville Fencing Academy (“Program”), I:
1. Acknowledge that I understand the nature of the Program and believe that I am qualified and in proper physical condition to participate in the Program. I further agree that if at any time, I believe conditions to be unsafe with respect to my physical condition, the equipment, or facilities, it shall be my responsibility to, and I will, immediately discontinue participation in the Program.
2. Further acknowledge that the program may involve the risk of bodily injury (including the possibility of permanent disability, paralysis, or death), which may be caused by (a) my own actions or inactions, (b) the actions or inactions of others participating in the Program, (c) the condition of the equipment and/or facilities at which the Program is located, or (d) the actions or inactions of the entities and persons identified below; and I fully accept and assume all such risk and all responsibility for losses, costs, and damages I may incur as a result of my participation in the Program.
3. Acknowledge that this is a program sponsored by the Bronzeville Fencing Academy and as such, all applicable regulations, policies, procedures and consequences as defined in the participant handbook will apply during my participation in this program.
4. Acknowledge that information about me provided in this registration may be used by a Bronzeville Fencing Academy representative or any individual or organization identified by Bronzeville Fencing Academy as needed in order to effectively execute this program.
5. Accordingly, I hereby release the Bronzeville Fencing Academy, together with their directors, officers, employees, volunteers, and agents from all liability, claims, demands, losses, or damages arising out of my participation in the Program; and I further agree that if, despite this release and waiver of liability agreement I, or anyone on behalf of me, makes a claim released in this agreement, I will indemnify and hold harmless each entity and person released herein from any and all litigation expenses, attorney fees, loss, liability, damage, or cost they may incur as the result of such claim.
6. I have read this agreement as well as all Bronzeville Fencing Academy regulations, policies, procedures and consequences as defined in the handbook, fully understand its terms, and have voluntarily entered into this agreement of my own free will based only upon the terms and conditions included herein.
MEDIA CONSENT
I hereby give Bronzeville Fencing Academy (“Academy”) the absolute and irrevocable rights to use my name, quotes and/or photos and images on the Internet (World Wide Web), in print publications, video and multimedia presentations, and/or for any purpose which may include, but not limited to display, public relations, marketing, or designs. This agreement is a permanent licensing agreement that allows the use of any images, quotes and/or my name for any publishing purposes in the promotion of the Academy. I agree to hold harmless the Board of Directors and/or the Academy from any and all actions, claims, and demands a rising out of or in connection with the use of all or any part of the photographs (including computer images or reproductions of any kind), including any editorial or comment which may accompany the images in their displayed format and/or my name. I will not hold the Academy liable for any errors, negligence, or gross negligence, in the editing or displaying of said images, quotes and/or in the use of my name.
CONSENT TO PARTICIPATE AND MEDICAL WAIVER
I am, an Adult (“I”) who has enrolled in a Bronzeville Fencing Academy program. In return for being accepted into a program being offered by the Bronzeville Fencing Academy (“Program”), I:

Do hereby acknowledge that the program may involve the risk of bodily injury (including the possibility of permanent disability, paralysis, or death), which may be caused by (a) my own actions or inactions, (b) the actions or inactions of others participating in the Program, (c) the condition of the equipment and/or facilities at which the Program is located, or (d) the actions or inactions of the entities and persons identified below; and I fully accept and assume all such risk and all responsibility for losses, costs, and damages I may incur as a result of my participation in the Program.
2. Agree that in an emergency, any Bronzeville Fencing Academy representative may transport or authorize the transportation of my person to a hospital/medical facility and I authorize any physician or other medical personnel to carry out any diagnostic procedures or emergency care deemed necessary. I understand that the cost of medical attention and ambulance are my responsibility.
IN WITNESS WHEREOF, the undersigned certifies by signing below, that I am of legal age, 18 years of age or older or that I am the parent or legal guardian of the adult herein identified. I have read this agreement and fully understand the contents herein.
